Coordination Case Manager, Home Care Escalations
Overview
You own the escalated and at-risk case queue at a 50-year licensed home care agency, working cases end-to-end until they stabilize. This seat handles the hardest cases that need a real owner, not a queue. You partner with Coordinators, Clinical, Compliance, and Billing to resolve authorization disputes, eligibility questions, and workplace-relations gaps. You document every escalation to managed-care standards and close the loop so fixes stick. This role reports directly to the Director of Coordination and operates on EOS.
What You'll Do8
- 1Own the escalated and at-risk case queue, working cases end-to-end until they are stable and ready to hand back.
- 2Resolve authorization disputes and eligibility questions with Clinical and Billing to maintain continuity of care.
- 3Drive complex client and aide matching, stepping in where continuity has already broken once.
- 4Document every escalated file to program and managed-care standards using HHAeXchange.
- 5Record every escalation outcome in the case record and hand back to the assigned Coordinator.
- 6Surface problems early and work cross-functionally with Clinical, Compliance, and Billing.
- 7Communicate clearly with clients, families, aides, and internal partners, including difficult conversations.
- 8Operate within a structured EOS system to ensure consistent process and follow-through.
Requirements7
- 12+ years of home care coordination, case management, or care management experience.
- 2Fluency in HHAeXchange or a comparable scheduling/EVV platform.
- 3Comfort with ambiguity, able to pick up a half-resolved case and work it to closure.
- 4Clear, calm communication with clients, families, aides, and internal partners.
- 5Comfort in a structured operating system, specifically EOS.
- 6Nice to have: lead or supervisor experience, exposure to DFTA, NHTD, CDPAP, or managed long-term care.
- 7Bilingual in any language spoken by the client population.
