What You'll Do5
- 1Assist clients with daily living activities, medication management, and companionship in their homes.
- 2Build positive relationships with clients, families, and staff to ensure clear communication and high-quality care.
- 3Document care delivered accurately and report changes or concerns to the clinical team.
- 4Participate in ongoing training to stay current with best practices and regulatory requirements.
- 5Collaborate with the clinical team to implement individualized care plans tailored to each client's needs.
Requirements7
- 1CNA certification or equivalent training (e.g., HHA or PCA) required.
- 21+ year of experience in home care or a similar setting preferred.
- 3Valid driver's license and reliable transportation to travel to client homes.
- 4Pass a background check and drug screening as required by state regulations.
- 5Ability to lift and transfer clients up to 50 lbs as needed.
- 6Strong communication and interpersonal skills for working with clients and families.
- 7Compassionate and patient demeanor with a commitment to dignity and respect.
