Care Manager Guardianship
Overview
Case Manager leads elderly client services within Guardianship Program. Reports to Co-Directors of Guardianship. Provides meaningful impact supporting families and individuals with dementia or Alzheimer’s disease. Combines field work with collaborative teamwork.
What You'll Do7
- 1Coordinate services for elderly clients in Guardianship Program
- 2Collaborate with families courts hospitals social services agencies nursing homes
- 3Conduct regular field visits to assess needs
- 4Develop and monitor individualized care plans
- 5Maintain accurate documentation compliant with program requirements
- 6Advocate for clients to secure necessary resources
- 7Participate in team meetings trainings and case reviews
Requirements6
- 1Bachelor’s degree in Social Work or related discipline
- 2New graduates encouraged to apply
- 3Bilingual English Spanish preferred
- 4Proficiency with Microsoft Office Suite
- 5Valid driver’s license and personal vehicle
- 6Experience with case management software
