Care Coordinator (Agave)
Overview
The Care Coordinator for Agave will conduct ongoing tenant engagement to encourage participation in support services under a Housing First model. Own tenant engagement initiatives and build trust through relationship development. Establish connections between tenants and community resources addressing housing financial social mental health and physical health needs. Support community building via social activities and educational opportunities. This role differs by focusing on personalized service planning and advocacy for tenant stability.

- 2Develop individualized service plans aligned with tenant goals
- 3Document progress and strengths in regular notes
- 4Transport tenants to essential appointments
- 5Assist with lease adherence and address violations
- 6Coordinate recreational social activities and educational programs
- 7Prepare documentation for HUD compliance
- 8Maintain accurate client records in HMIS and CHAMP
- 9Organize records following USHS QA standards
- 10Drive tenant retention through proactive support
- 11Manage transportation logistics for client visits
Requirements10
- 15+ years experience supporting individuals with complex mental health or substance use conditions
- 2Proficiency in Microsoft Word Excel and PowerPoint
- 3Valid driver’s license and access to a registered vehicle
- 4Familiarity with Los Angeles County housing resources
- 5Knowledge of substance abuse and mental health issues
- 6Ability to navigate CHAMP and HMIS databases
- 7Strong written and oral communication skills
- 8Detail oriented and organized
- 9Experience with case management software
- 10Bilingual English Spanish proficiency preferred
