Overview

You will own behavioral health coordination at Macdonald Residence, a 24/7 supportive housing site in Portland serving residents with complex needs. Working with the Director of Behavioral Services, you will craft and implement Behavior Support Plans, de-escalate crises, and lead resident groups. You will join a multidisciplinary team spanning Nursing, Direct Care, and Resident Services, coordinating external providers and community resources. This role offers full-time Sunday-Thursday swing hours, on-site work, and the chance to directly shape resident wellbeing in a trauma-informed environment.

What You'll Do8

  • 1Build and update Behavior Support Plans for residents, coordinating with Nursing and Resident Services to address behaviors like substance use, aggression, and dementia.
  • 2De-escalate residents in crisis using verbal techniques, ensuring safety and dignity on the Swing shift.
  • 3Facilitate behavioral health groups on-site and with community agencies, adapting content to resident needs.
  • 4Document incidents and follow-up daily, creating weekly trend reports to guide care adjustments.
  • 5Lead resident admissions by evaluating needs, drafting Behavior Support Plans, and reviewing history.
  • 6Train staff on behavioral health topics in partnership with the Director of Behavioral Services.
  • 7Coordinate with external providers and resources to ensure comprehensive care continuity.
  • 8Support resident housing stability by assisting with apartment cleaning and accompanying to community appointments.

Requirements8

  • 1High School Diploma or equivalent with 3+ years in behavioral health or related field.
  • 2CADC I or QMHA certification helpful, but not required.
  • 3Proven crisis de-escalation and conflict resolution skills in high-stress settings.
  • 4Strong writing and documentation skills, including Electronic Medical Records (EMR) proficiency.
  • 5Familiarity with community support systems, healthcare, and legal resources.
  • 6Ability to work independently as sole behavioral health staff on-site.
  • 7Physical capability to lift 25 lbs and perform tasks like stooping and kneeling.
  • 8Commitment to confidentiality and professional boundaries.
