Overview

As Assistant Wellness Director at StoryPoint, you will own clinical care delivery for 100+ seniors across assisted living and memory care, enforcing our 1440 care standard. You will lead a team of 15-20 care staff, set clear expectations, and drive performance through regular feedback and recognition. This role integrates clinical oversight with operational leadership, requiring daily coordination with families, pharmacy, and nursing staff. You will also conduct quarterly resident evaluations and care conferences, using data to set measurable goals. LPN or RN licensure is required, with supervisory experience preferred. You will be the clinical backbone, ensuring right care at the right price with the right staff.

What You'll Do10

  • 1Lead the wellness team, setting expectations and holding staff accountable for 1440 care standards.
  • 2Perform admission and quarterly resident evaluations, adjusting care plans after hospital returns or condition changes.
  • 3Verify MARS and TARS documentation, coordinating with pharmacy to ensure accurate medication administration.
  • 4Review incident reports, initiating assessments and care conferences for existing residents.
  • 5Communicate emergency policies, updating staff on procedures and conducting training sessions.
  • 6Schedule resident health education sessions, covering topics like fall prevention and nutrition.
  • 7Drive employee engagement by giving real-time feedback and recognizing staff contributions weekly.
  • 8Oversee resident and family communication, providing clear updates on care plans and changes.
  • 9Collaborate with the Director on revenue and staffing, aligning care costs with resident needs.
  • 10Maintain flexible availability for on-call, weekends, and holidays to ensure continuous coverage.

Requirements10

  • 1LPN or RN license registered in the State of Ohio.
  • 22+ years of experience in assisted living or memory care settings.
  • 31+ year supervisory experience, managing clinical or care teams.
  • 4Proven ability to read and interpret safety rules and procedure manuals.
  • 5Strong written and verbal communication for reports and family interactions.
  • 6Basic math skills for medication dosages and care documentation.
  • 7Demonstrated problem-solving in complex clinical situations.
  • 8Ability to stand for long periods and lift up to 25 pounds.
  • 9Flexibility to work weekends, holidays, and on-call shifts.
  • 10Compassionate demeanor with focus on resident dignity and family peace of mind.
