Assessment Nurse Remote Hybrid HF Management Services
Overview
The Assessment Nurse will conduct home and virtual evaluations for members across downstate New York. This hybrid role combines remote work with in‑person visits. You will own member enrollment and drive care decisions. The position offers flexible scheduling and competitive compensation.
What You'll Do11
- 1Perform home and virtual assessments using the Uniform Assessment System to determine service needs
- 2Make enrollment and continued eligibility determinations based on clinical expertise
- 3Conduct in‑person Care Management visits and liaise with CMTs and home care agencies
- 4Provide community‑based training for frail populations and self‑care techniques
- 5Review nursing assessments to update Care Plans and support nursing home admissions
- 6Deliver grassroots outreach and coordinate multidisciplinary care planning
- 7Utilize electronic PHI databases and tools such as UAS and CMT
- 8Travel within downstate New York to meet members and attend required meetings
- 9Collaborate with pediatric and geriatric specialists to tailor care solutions
- 10Maintain accurate documentation and communicate findings to stakeholders
- 11Support workforce development through mentorship and continuous learning
Requirements11
- 1Current New York State RN license and willingness to travel throughout downstate New York
- 2Minimum 5+ years building ETL pipelines with Spark and Airflow
- 3Proven experience with UAS and CMT platforms
- 4Strong knowledge of health insurance, Medicaid, Medicare, and MLTCP
- 5Ability to handle heavy caseloads with effective time management
- 6Intermediate proficiency in Microsoft Word Excel and Outlook
- 7Familiarity with Spanish Russian French Creole Mandarin Cantonese languages
- 8Demonstrated commitment to patient‑centered care and family engagement
- 9Certification in Certified Professional Canteen (CPC) Card or equivalent
- 10Previous field experience assessing pediatric or geriatric populations
- 11Driver’s license and comfort driving in urban and suburban environments
