What You'll Do11
- 1Conduct comprehensive assessments of patients and families to identify hospice needs
- 2Obtain accurate demographic and medical histories including diagnoses
- 3Introduce agency to patients and caregivers while coordinating start-of-care visits
- 4Order necessary DME supplies medications and educate patients on hospice care
- 5Deliver community education to assisted living facilities and hospitals
- 6Provide bedside education on disease progression and end-of-life care
- 7Participate in discharge planning meetings to ensure safe home transitions
- 8Determine hospice eligibility and communicate findings to clinical teams
- 9Collaborate with staff to prevent unnecessary hospital readmissions
- 10Transfer patients using proper body mechanics to maintain safety
- 11Accept diverse lifestyles and cultural backgrounds
Requirements9
- 15+ years experience as a Registered Nurse in acute care or oncology settings
- 2Associate degree from an accredited nursing school required
- 32-3 years hospice experience preferred
- 4Proficiency with Microsoft Office Suite EPIC and HCHB systems
- 5Ability to travel within Kansas City and Leawood areas
- 6Strong communication skills for interacting with patients and families
- 7Experience with telehealth platforms and electronic health records
- 8Commitment to patient-centered care across diverse populations
- 9Drug screening clearance required prior to employment
