Speech Therapist for Home Health & Schools
Overview
You will own the full therapy cycle for a caseload of 30-40 patients at a community health center in Los Angeles: evaluate speech needs, design personalized treatment plans, and adjust goals as patients progress. You will join a team of 10 speech therapists and collaborate with occupational therapists and special educators. This contract role offers flexibility to build your own schedule and direct patient relationships.
What You'll Do6
- 1Evaluate new patients to assess speech, language, and swallowing needs, then design personalized treatment plans using Evidence-Based Practice.
- 2Implement treatment plans, adjusting goals and milestones based on patient progress and family input.
- 3Document every patient encounter in Electronic Medical Records within 24 hours.
- 4Coach patients and families on home exercises, using Telehealth tools for remote follow-up.
- 5Collaborate with the care team to align speech goals with IEPs and other therapy plans.
- 6Track patient outcomes and present monthly progress reports to the clinical supervisor.
Requirements6
- 1Active state license as a Speech Language Pathologist.
- 22+ years of clinical experience, including pediatric or geriatric settings.
- 3Proven skill in SIPT or PROMPT techniques.
- 4Strong charting and documentation skills using Microsoft Office and EHR systems.
- 5Excellent communication and interpersonal skills for patient and family education.
- 6Reliable transportation to travel between school and home health sites.
