Social worker, hospice care in skilled nursing facilities
Overview
Owns psychosocial care for hospice patients in SNFs and collaborate with interdisciplinary teams at a prominent not-for-profit hospice. This role delivers assessments, counseling, education, and discharge planning to patients and families, primarily in patient homes or facilities. Requirements10
- 1Bachelor's degree in social work from an accredited program and 3-5 years palliative care or hospice experience
- 2Ability to assess emotional and social needs and administer psychological assessments
- 3Excellent communication and interpersonal skills
- 4Ability to collaborate with interdisciplinary team and solve problems
- 5Proficiency with computers and documenting processes
- 6Knowledge of community resources and financial benefits
- 7Awareness of cultural diversity and professional boundaries
- 8Current Arizona driver's license and auto insurance
- 9Must be able to work independently and manage time effectively
- 10Bilingual abilities are a plus
