What You'll Do9
- 1Deliver personalized therapy sessions in homes and senior living communities
- 2Conduct comprehensive evaluations and design individualized care plans
- 3Implement therapeutic interventions focused on mobility and functional independence
- 4Maintain accurate documentation within FOX’s EMR system
- 5Collaborate with interdisciplinary teams to enhance patient outcomes
- 6Educate patients and caregivers on safety and aging strategies
- 7Provide telehealth services when appropriate to increase accessibility
- 8Manage and adjust personal treatment schedules independently
- 9Contribute to a supportive clinical culture emphasizing clinician leadership
Requirements5
- 15+ years building geriatric rehabilitation programs with Spark and Airflow
- 2CPC Card certification preferred
- 3AWS Certified cloud solutions experience valued
- 43-5 seasons of clinical experience
- 5Medical license in Massachusetts or eligibility to obtain
