Scheduler, Home Care & EMR Coordination
Overview
Home Care Scheduler will coordinate, track, and optimize scheduling for Home Care Assistants and field staff in a San Jose, CA environment. You will work under the Home Care Supervisor to ensure timely, high-quality in-home care while managing schedule changes, EMR authorizations, and care logs. Serve as a liaison between patients, families, staff, and vendors and deliver reliable service in a fast-paced setting. This role offers meaningful impact for vulnerable seniors and opportunities to collaborate with cross-functional teams.
What You'll Do10
- 1Build and adjust daily care schedules for home care assignments and communicate changes to patients, families, and staff
- 2Ensure EMR authorizations are in place for approved services and maintain accurate care logs and timesheets
- 3Maintain up-to-date records of scheduled visits including outcomes and reschedule reasons
- 4Implement reminder calls and pre-visit instructions to improve provider, staff, and patient experiences
- 5Maintain privacy and confidentiality of patient data and company procedures
- 6Collaborate with the Supervisor, Home Care Coordinators, and Home Services staff to manage workload and priorities
- 7Complete required training, education, and occupational health screenings in a timely manner
- 8Communicate proactively about workload and priorities, adapting to evolving business needs
- 9Follow policies, participate in quality improvement activities, and attend team meetings
- 10Perform other related duties as assigned
Requirements7
- 1High School Diploma or GED
- 2Minimum of 1 year of scheduler experience in a home health or home care agency
- 3Strong organizational, communication, and interpersonal skills
- 4Ability to prioritize tasks and manage staff schedules in a fast-paced environment
- 5Ability to work independently with minimal supervision
- 6Experience working with diverse populations and cultures
- 7Desire to support vulnerable seniors in a confidential setting
