As an RN in the Texas HCS program, you will own clinical oversight for individuals with IDD in a home-based setting. You will conduct initial and annual assessments, delegate tasks to unlicensed staff per Texas regulations, and monitor chronic conditions like diabetes and seizures. Working with a dedicated care team, you ensure compliance with physician orders and state standards. What You'll Do7
- 1Complete initial and annual nursing assessments for clients in the HCS program, documenting findings per HHSC guidelines.
- 2Delegate nursing tasks to unlicensed staff under 22 TAC Chapter 225, ensuring competency and legal compliance.
- 3Monitor chronic conditions including diabetes, seizures, and other IDD-related health issues, adjusting care as needed.
- 4Develop and update nursing service plans in collaboration with the care team and physician orders.
- 5Train unlicensed staff on medication administration, emergency protocols, and client-specific care plans.
- 6Ensure all documentation meets Medicaid waiver and HCS regulatory standards.
- 7Communicate with physicians, families, and team members to coordinate comprehensive care.
Requirements7
- 1Active RN license from the Texas Board of Nursing or compact state.
- 21+ years of nursing experience within the last 36 months.
- 3Pass criminal history background checks required by Texas Health and Human Services (HHSC).
- 4Experience with IDD (Intellectual and Developmental Disabilities) preferred.
- 5Familiarity with Medicaid waiver programs and HCS regulations highly desirable.
- 6Ability to conduct comprehensive assessments and develop nursing service plans.
- 7Valid Texas Driver's License and CPR/BLS certification.
