RN Case Manager, No Travel Daylight
Overview
Provide skilled nursing care and case management at a single senior living facility in Pittsburgh. No travel, steady Monday-Friday daylight schedule, and a small team of nurses and aides. RN license holders with Oasis experience will find a stable role with full benefits and free meals.
What You'll Do8
- 1Perform patient evaluations and routine skilled nursing visits within the assisted living campus.
- 2Document and update visit notes into Brighttree within 24 hours of each visit.
- 3Coordinate with physicians, families, and staff to adjust care plans as patient conditions change.
- 4Administer medications, wound care, and other nursing interventions per protocol.
- 5Educate patients and families on disease management, medication, and post-discharge care.
- 6Audit and maintain accurate patient records to ensure compliance with state and federal guidelines.
- 7Lead care plan meetings and advocate for patient needs with the interdisciplinary team.
- 8Monitor vital signs and report changes to the supervising physician promptly.
Requirements7
- 1Active RN license in Pennsylvania
- 21+ year experience as a Home Care Nurse
- 3Oasis experience preferred, but not required
- 4Reliable transportation and proof of insurance
- 5Ability to pass a background check
- 6CPR certification
- 7Comfortable working with geriatric population
