Registered Nurse (RN) Home Health Job in Canton, GA
Overview
You will own patient care for a caseload of home health clients in Canton, GA, delivering skilled nursing in their homes. You will assess, plan, and implement care using EPIC and HomeCare HomeBase, coordinating with a multidisciplinary team. What You'll Do7
- 1Complete comprehensive patient assessments within 48 hours of admission, then build and update individualized care plans in HomeCare HomeBase.
- 2Administer medications, IV therapy, and skilled nursing interventions per physician orders, ensuring safety and efficacy.
- 3Monitor and record patient vitals and condition changes, escalating to physicians when needed.
- 4Coordinate care with physicians, therapists, and other team members through daily huddles and shared documentation.
- 5Educate patients and families on disease management, medication regimens, and home safety strategies.
- 6Perform wound care, catheter care, and other skilled procedures, following infection control protocols.
- 7Document every visit in EPIC within 24 hours to meet regulatory standards and support billing.
Requirements6
- 1Active RN license in Georgia
- 21-2 years of clinical nursing experience; home health preferred
- 3Current CPR/BLS certification
- 4Valid driver's license and reliable transportation for home visits
- 5Strong clinical assessment and critical thinking skills
- 6Excellent communication and time management abilities
