Registered Nurse, Home Health Care & EMR Experience
Overview
Registered Nurse at BAYADA Home Health Care will lead 1:1 home health visits across designated territories in Faifax, VA and surrounding areas. This role requires a current RN license, at least 1 year of clinical experience, and prior home health preferred. What You'll Do9
- 1Build and execute patient care plans during home visits in assigned territories
- 2Monitor patient conditions and report changes to the Clinical or Client Services Manager
- 3Administer medications, perform wound care, and manage treatments
- 4Document observations, interventions, and evaluations using EMR tools
- 5Coordinate case management and follow up on doctors' orders
- 6Perform client assessments as needed and ensure proper documentation
- 7Travel to assigned cases and maintain timely communication with the care team
- 8Ensure adherence to established policies, regulations, and quality standards
- 9Provide education and support to patients and families during visits
Requirements9
- 1Current RN license in STATE
- 2Minimum of 1 year of recent, verifiable clinical experience
- 3Prior home care experience preferred
- 4Graduation from an accredited nursing program
- 5Ability to work independently and manage time effectively
- 6Strong interpersonal and computer skills with EMR experience preferred
- 7Ability to travel to cases as assigned
- 8Excellent documentation and communication skills
