Overview

Nellis AFB seeks a Registered Nurse - Discharge Planner leading discharge planning for inpatients same day surgeries and emergency department patients. You will triage urgent needs develop repeatable processes predict discharge requirements coordinate multidisciplinary transitions and ensure timely safe discharges aligned with DoD/W standards.

  • 4Facilitating smooth transitions to appropriate care levels
  • 5Collaborating with social workers and healthcare teams for comprehensive discharge
  • 6Coordinating transfers to Mike O’Callaghan Military Medical Center and other facilities
  • 7Surveying continuum of care and reporting quality concerns
  • 8Participating in committees and process improvement teams
  • 9Educating patients providers and staff on post-discharge care
  • 10Establishing partnerships with third-party payers and TRICARE MCSC
  • 11Validating insurance benefits and ensuring proper referrals
  • 12Providing ongoing education on discharge planning processes
  • 13Training new personnel and maintaining strong interdisciplinary relationships
  • 14Using appropriate communication formats and documenting activities
  • 15Maintaining safe work environment and adhering to regulatory policies

Requirements11

  • 1Bachelor of Science in Nursing from NLNAC or CCNE accredited program
  • 2Minimum 5 years total nursing experience with last 36 months in direct patient care hospital discharge planning
  • 3Extensive experience in discharge planning for at least 24 consecutive months
  • 4Knowledge of EMTALA and COBRA regulations
  • 5Current AHA CPR BLS and Neonatal Resuscitation Program certification
  • 6Active unrestricted RN license from any state
  • 7Ability to pass government background check and security clearance
  • 8Proficiency in using Interqual Milliman or other evidence based programs
  • 9Understanding of TRICARE managed care protocols
  • 10Experience with DME and medication authorization processes
  • 11Strong leadership and communication skills across diverse healthcare professionals
