Overview

You will own discharge planning and care coordination for acute care patients at a Bronx hospital, managing a caseload of 15-20 patients per day. You will work with an interdisciplinary team of physicians, social workers, and RN staff to ensure safe, timely transitions. BSN and RN licensure are required, with CarePort, MCG, or InterQual preferred. This 13-week travel assignment offers 37.5 guaranteed hours per week and a competitive pay package, with PRI and SCREEN certifications mandatory.

What You'll Do9

  • 1Coordinate discharge planning for a caseload of 15-20 acute care patients, using CarePort to track progress and identify barriers.
  • 2Conduct comprehensive assessments to determine post-acute care needs, leveraging MCG and InterQual criteria for utilization review.
  • 3Lead interdisciplinary team rounds on a daily basis, presenting patient cases and aligning on discharge plans with physicians and social workers.
  • 4Complete PRI assessments for all patients requiring skilled nursing placement, ensuring accuracy within 24 hours of referral.
  • 5Execute SCREEN evaluations for psychiatric and substance abuse patients, coordinating with mental health teams for safe discharge.
  • 6Facilitate patient education on post-discharge instructions, covering medication, follow-up appointments, and community resources.
  • 7Review and approve patient charts for insurance authorization, submitting documentation to payers via Utilization Review processes.
  • 8Serve as the point of contact for families, updating them on discharge plans and addressing concerns to prevent delays.
  • 9Work with community-based agencies to arrange home health services or equipment, ensuring patients leave with necessary support.

Requirements7

  • 1Active RN license in New York State with BSN degree.
  • 21+ years of inpatient case management experience in an acute care setting.
  • 3BLS certification from the American Heart Association.
  • 4PRI certification for patient review instruments.
  • 5SCREEN training certification for psychiatric and substance abuse assessments.
  • 6Experience with CarePort, MCG, or InterQual in utilization review.
  • 7Prior critical care experience preferred but not required.
