What You'll Do9
- 1Lead patient care planning and execution to improve outcomes within Home Health caseloads
- 2Coordinate with interdisciplinary teams to ensure timely, high-quality visit documentation and patient education
- 3Travel to patient homes and agency locations to support care delivery and continuity
- 4Document care activities, assessments, and progress in electronic medical records
- 5Engage families and caregivers to support patient adherence and safety
- 6Monitor and adjust care plans based on patient needs and regulatory requirements
- 7Participate in on-call rotation to ensure 24/7 patient support
- 8Contribute to staff training and mentorship to uphold clinical standards
- 9Support quality improvement initiatives and compliance with policies
Requirements5
- 1Graduate from an approved school of professional nursing and currently licensed in the state of operation
- 2Active RN license in IL
- 3One (1) year RN experience in home health or related setting
- 4Valid CPR certification
- 5Licensed driver with insured vehicle in good working order and ability to travel to all locations
