Nurse Case Manager - Care Management & Population Health
Overview
You will own care management for an assigned patient panel, driving comprehensive risk assessments and personalized care plans at Chicago Family Health Center. You will coordinate transitions of care and collaborate with providers and care teams to ensure continuity. This role requires clinical expertise and a community-based mindset, with a focus on empowering patients toward self-management. What You'll Do7
- 1Manage ongoing care management for an assigned patient case load, focusing on population health outcomes.
- 2Perform comprehensive risk assessments and develop individualized patient care plans for all assigned cases.
- 3Coordinate patient access to care, overseeing efforts of collaborating Care Coordinators and referral staff.
- 4Address all relevant transitions of care when patients obtain hospital-based care, ensuring seamless follow-up.
- 5Collaborate actively with patient care teams to improve care quality and patient satisfaction.
- 6Deliver face-to-face patient education and telephone follow-up based on care management guidelines.
- 7Promote self-care management and assist patients with navigating the healthcare system.
Requirements8
- 1Associate degree in Nursing from an accredited college or university required.
- 2Bachelor's degree in Nursing required for complex disease care management.
- 3Valid LPN or RN license in the state of Illinois.
- 4Current CPR certification.
- 52-3 years of experience as a qualified Case Manager preferred.
- 6Demonstrated experience working in a community-based environment.
- 7Experience working with highly complex care packages in health or social care settings.
- 8Bilingual ability preferred.
Salary Insight
$60 - $94k per year
