What You'll Do9
- 1Design and deliver personalized care plans for medically complex patients
- 2Perform ventilator management and troubleshooting
- 3Lead patient education and family training sessions
- 4Collaborate with interdisciplinary teams to coordinate care
- 5Maintain accurate documentation in electronic health records
- 6Develop treatment protocols aligned with physician recommendations
- 7Train new hires on clinical best practices
- 8Monitor patient progress and adjust care plans as needed
- 9Ensure compliance with safety and regulatory standards
Requirements10
- 15+ years clinical experience in home health settings
- 2AWS Certified Cloud Practitioner credential
- 3CPC Card certification required
- 4Spark and Airflow proficiency demonstrated
- 51 year minimum clinical experience
- 6Current IL Registered Nurse license verified
- 7CPR certification (AHA Healthcare Provider) current
- 8Bachelor's degree in Nursing or related field
- 9License to practice in Illinois
- 10Ability to pass background check
