Overview

Pulmonology care at scale within a PHM program across long-term care facilities in the Dallas area. You will own on-site assessments, diagnostics, and treatment plans for residents with asthma, COPD, pneumonia, and related pulmonary disorders. Collaborate with a team of advanced practice providers and clinicians to drive proactive respiratory care and improve patient outcomes. This role emphasizes on-site visits, travel to multiple nursing homes, and a flexible schedule to cover facilities.

What You'll Do10

  • 1Build comprehensive care plans for residents with respiratory diseases and adjust them based on progress
  • 2Diagnose and treat common respiratory illnesses including asthma, bronchitis, and pneumonia
  • 3Prescribe inhalers, antibiotics, and pulmonary rehabilitation programs and monitor responses
  • 4Conduct spirometry, chest X-rays, and arterial blood gas analyses to inform treatment decisions
  • 5Educate patients and families on disease management, medication adherence, and lifestyle modifications
  • 6Travel regularly to nursing homes within the region to deliver on-site care and ensure coverage
  • 7Collaborate with the PHM team to optimize patient outcomes and care coordination
  • 8Maintain accurate documentation in EMR and ensure compliance with facility policies
  • 9Develop individualized schedules to balance patient needs with travel requirements
  • 10Stay current on pulmonology best practices and participate in continuous learning opportunities

Requirements11

  • 1Active and unrestricted NP PA or MD license
  • 2DEA license and CPR BCLS/BLS certification
  • 3Master's degree in nursing and graduation from an accredited NP program
  • 4Minimum 6 months of experience in long-term care or skilled nursing facilities
  • 5At least 1 year of experience in pulmonology or respiratory care
  • 6Local travel throughout Texas (100) required
  • 7Reliable transportation and willingness to travel to multiple nursing homes
  • 8Documenting EMR experience preferred with training provided
  • 9Strong organizational and time management skills
  • 10Ability to work in a collaborative, mission-driven environment
  • 11Must be able to travel on-site to facilities as required
