Overview

You will execute physical therapy treatment plans for adult patients in their homes across Pitt and Lenoir Counties, NC, working under a Licensed Physical Therapist (PT). You will own the full treatment cycle: prescribing exercises, instructing families on assistive devices, and documenting every visit in McKesson EMR. This role stands out because you will work autonomously in the field while collaborating daily with RNs and directors, directly restoring function and independence for patients in their familiar surroundings.

What You'll Do9

  • 1Execute physical therapy treatment plans prescribed by the supervising PT to relieve pain and restore function and independence in adult home health patients.
  • 2Instruct patients and families on the safe use of canes, walkers, and wheelchairs, plus prosthetic and orthotic devices, following the PT's direction.
  • 3Observe and record patient responses to treatment, reporting changes to the supervising PT and clinical staff on the same day.
  • 4Document each visit with timely progress notes in McKesson EMR, ensuring notes are downloaded and communicated twice daily.
  • 5Collaborate with the supervising PT to manage caseloads and meet weekly productivity goals, adjusting schedules as patient needs shift.
  • 6Communicate daily with the interdisciplinary team, including RNs and directors, to coordinate care and scheduling across home visits.
  • 7Attend all required staff meetings, rehab meetings, and in-service programs to stay current with agency protocols.
  • 8Maintain full compliance with in-service training hours and the agency's personnel policies and Compliance Plan.
  • 9Orient new staff as needed and take on additional duties assigned by the supervisor to support team goals.

  • 1Current and valid North Carolina Physical Therapist Assistant (PTA) license.
  • 2Minimum of one (1) year of recent, verified Adult Home Health experience.
  • 3Experience executing treatment plans under the direct supervision of a Licensed Physical Therapist (PT).
  • 4Proficiency in using McKesson or similar EMR systems for timely documentation.
  • 5Ability to record patient conditions accurately and communicate changes to supervising staff.
  • 6Skill in instructing patients on assistive devices like canes, walkers, and wheelchairs.
  • 7Commitment to maintaining timely and accurate patient records with daily progress notes.
  • 8Reliable transportation for travel to patient homes across Pitt and Lenoir Counties, NC.
