What You'll Do8
- 1Manage full-scope Primary Care for a geriatric population, 12-18 patients per day.
- 2Diagnose and treat common conditions, coordinating care to prevent hospitalizations.
- 3Collaborate with support staff to ensure efficient patient flow and follow-up.
- 4Document patient encounters in EHR within 24 hours.
- 5Prescribe medications and manage chronic conditions like diabetes and hypertension.
- 6Participate in care coordination meetings with the interdisciplinary team.
- 7Adapt to flexible schedules, Monday-Friday 8am-5pm, or 4 days per week.
- 8Start patient care within 30 days of credentialing, with expedited onboarding.
Requirements8
- 1Board Certified in ABIM or ABFM.
- 2Active CO medical license.
- 3Active DEA in CO or willing to transfer.
- 4BLS certification.
- 52+ years outpatient Primary Care experience, preferably with Medicare Advantage.
- 6Comfortable with a geriatric patient population (65+).
- 7Willing to commit to a 3-month minimum locums assignment.
- 8Open to locums-to-perm transition.
