What You'll Do10
- 1Manage a panel of adult patients in an outpatient primary care setting, providing comprehensive assessments and treatment plans.
- 2Diagnose and treat acute and chronic conditions, emphasizing preventive care and patient education.
- 3Coordinate care with a multidisciplinary team, including physicians, nurses, and support staff, to optimize patient outcomes.
- 4Prescribe medications and therapies within scope of practice, ensuring compliance with state regulations and DEA requirements.
- 5Document patient encounters accurately in the electronic health record, maintaining high clinical standards.
- 6Participate in quality improvement initiatives aimed at improving health outcomes in the Bronx community.
- 7Engage in limited phone call rotation, providing clinical support to patients and staff after hours.
- 8Collaborate with community resources to address social determinants of health affecting patient populations.
- 9Mentor and support clinical staff, fostering a collaborative and educational environment.

Requirements8
- 1Graduate of an accredited Nurse Practitioner program in Family or Adult-Gerontology Primary Care.
- 2Board Certified or Board Eligible as a Nurse Practitioner.
- 3Active or eligible New York State Nurse Practitioner license.
- 4DEA registration or ability to obtain upon hire.
- 5Strong clinical skills in primary care, including chronic disease management and preventive care.
- 6Excellent communication and interpersonal skills, with ability to build rapport with diverse patients.
- 7Commitment to value-based care and improving health outcomes in underserved communities.
- 8Ability to work full-time, Monday to Friday, with participation in limited phone call rotation.
