Physician Assistant, Internal Medicine & Geriatrics in Phoenix
Overview
Own patient care across medically complex geriatric populations within a physician-led primary care practice. You will develop strong clinical autonomy in a relationship-based setting serving assisted living, independent living, and memory care communities. Collaborate with MAs, scribes, and staff to coordinate care, while building long-term clinical judgment. This role emphasizes chronic disease management, meaningful patient relationships, and evolving autonomy.
What You'll Do6
- 1Build and manage care plans for medically complex geriatric patients
- 2Coordinate with families, specialists, hospice, home health, and facility staff to ensure continuity of care
- 3Manage chronic diseases including diabetes, CHF, COPD, hypertension, and dementia
- 4Oversee medication management and preventive care for long-term patient panels
- 5Document, order, and coordinate care across multiple facilities and teams
- 6Provide relationship-based medicine and contribute to a collaborative, physician-led environment
Requirements5
- 1Arizona PA license eligible within upcoming months
- 2NCCPA certification or scheduled/eligible to take PANCE within the next 3 months
- 3Strong academic and clinical foundation in primary care or geriatrics
- 4Excellent communication and interpersonal skills
- 5Interest in autonomous, relationship-driven medicine
