Physical Therapist, Per Diem - PACE Program
Overview
You will own participant assessments and treatment plans at Huron Valley PACE, a Program of All-Inclusive Care for the Elderly. You will develop evidence-based plans using therapeutic exercise, gait training, and pain management for participants with complex needs. Physical Therapy expertise drives care in clinic and community settings. You will collaborate with an interdisciplinary team to deliver coordinated care, lead group exercise programs, and adapt plans to individual goals. Work per diem with flexible days, achieving measurable outcomes in the first 90 days.
What You'll Do9
- 1Conduct initial and ongoing assessments of physical condition and functional status, completing them before monthly care plan meetings.
- 2Build and update Plans of Care with specific goals and treatment programs, documenting in the EMR system.
- 3Lead therapeutic exercise and gait training sessions, adjusting plans based on participant progress.
- 4Supervise program aides and staff implementing physical therapy programs, ensuring quality and safety.
- 5Recommend adaptive equipment, specialized shoes, and prostheses in collaboration with the Interdisciplinary Team.
- 6Participate in case conferences and staff meetings, communicating participant updates for coordinated care.
- 7Support discharge planning for participants transitioning from hospitals, assisted living, or skilled nursing facilities.
- 8Train family members and team members on therapeutic techniques and home exercise programs.
- 9Drive quality improvement initiatives to enhance service delivery and participant outcomes.
Requirements8
- 1Bachelor's or Master's degree in Physical Therapy.
- 22+ years experience as a Physical Therapist.
- 3Experience in outpatient, home health, or PACE setting preferred.
- 4Valid CPR certification or willingness to obtain.
- 5Adaptability in a clinical environment, including experience with EMR systems.
- 6Reliable transportation and valid driver's license.
- 7Strong communication, collaboration, and time-management skills.
- 8Ability to lift and move objects up to 35 pounds.
