What You'll Do11
- 1Assess and evaluate therapeutic functional status and develop care plans
- 2Direct physical therapy treatment and instruct patients and caregivers
- 3Determine priority needs and report patient reactions to treatment
- 4Initiate therapy programs and educate caregivers on goals
- 5Train patients in prosthetic device usage
- 6Identify additional home health services and refer as needed
- 7Prepare clinical progress summaries and participate in discharge planning
- 8Provide physical therapy consultation to home families and caregivers
- 9Supervise physical therapy assistants according to policy
- 10Conduct biweekly evaluations of home health aide care
- 11Maintain CPR certification and possess a valid driver
Requirements8
- 1Degree from accredited physical therapy program
- 2Current physical therapist license within the state
- 3Two years of relevant experience preferred
- 4Community/home health experience preferred
- 5Strong verbal and written communication skills
- 6Current CPR certification
- 7Reliable transportation vehicle in good working order
- 8Ability to commute reliably to Thornton CO location
