- 2Set and manage care goals that promote recovery and independence, adjusting plans based on patient progress.
- 3Evaluate each home environment for safety and mobility needs, recommending modifications to prevent falls.
- 4Document patient assessments, interventions, and outcomes in compliance with Florida home health regulations.
- 5Coordinate with physicians, nurses, and caregivers to ensure integrated care plans.
- 6Educate patients and families on exercise programs and safe movement techniques.
- 7Monitor patient responses to therapy and modify treatments as needed to achieve optimal outcomes.
- 8Participate in team meetings and contribute to quality improvement initiatives.
Requirements7
- 1Graduate of an accredited PT program with current PT license in Florida.
- 21+ years of experience required, with home health experience preferred.
- 3Valid Florida driver's license, active vehicle insurance, and clear MVR.
- 4Sound judgment and a passion for patient care and outcomes.
- 5Ability to perform physical tasks, including lifting and transferring patients.
- 6Strong communication and documentation skills.
- 7Must be eligible to work in the United States without sponsorship.
