What You'll Do10
- 1Build and deliver skilled PTA services to residents to improve safety and independence within SNF settings
- 2Collaborate with the interdisciplinary team to update goals and outcomes and document progress in Net Health
- 3Design therapy plans that align with resident goals and facility protocols to drive quality of life improvements
- 4Ship functional mobility and ADL training to reduce fall risk and enhance daily living activities
- 5Lead individual and small-group sessions using evidence-based techniques in a modern rehab gym
- 6Drive adherence to patient care plans and regulatory requirements to ensure compliant care delivery
- 7Own documentation accuracy and timely communication with care teams to support resident progress
- 8Scale therapy services across multiple facilities while maintaining high standards of care
- 9Debug and refine care plans based on resident response and clinician feedback
- 10Provide compassionate, resident-centered care with a focus on outcomes within a SNF setting
Requirements5
- 1Graduate of an accredited PTA program
- 2Active Florida PTA license or license-eligible
- 3SNF or post-acute experience preferred
- 4Net Health documentation knowledge preferred
- 5Must meet background screening requirements as per Florida regulations
