The Perioperative Optimization Nurse Practitioner or Physician Assistant leads patient care across the surgical episode for Medicare beneficiaries. What You'll Do11
- 1Conduct seven patient visits daily via in-person or virtual formats
- 2Provide direct care and coordinate treatment for geriatric patients undergoing orthopedic cardiac and major bowel surgeries
- 3Perform pre-operative assessments and optimize patients before surgery
- 4Deliver comprehensive perioperative support and post-discharge follow-up
- 5Lead risk mitigation and quality gap identification across care settings
- 6Serve as clinical resource for care pathway refinement and mentorship
- 7Collaborate with physicians and interdisciplinary teams to improve outcomes
- 8Manage panel of medium- and high-risk patients across multiple locations
- 9Conduct bedside procedures and write prescriptions per policy
- 10Document all encounters accurately and maintain compliance
- 11Participate in team meetings and contribute to program development
Requirements11
- 15+ years experience in primary care hospital medicine geriatrics perioperative medicine
- 2Master's Degree in Nursing required
- 3Advanced Practice Registered Nurse [APRN-NP - State License]
- 4Current license in Massachusetts
- 5Minimum 2 years of licensed NP experience
- 6Certification in CPC Card or related specialties preferred
- 7Proven ability to lead complex care initiatives
