What You'll Do11
- 1Provide direct patient care according to evidence based treatment guidelines
- 2Deliver team centered care while engaging patients and families in health decisions
- 3Offer counseling education and wellness resources on prevention and health maintenance
- 4Refer patients to specialty care and educational training as needed
- 5Utilize disease registries to identify care gaps and implement support services
- 6Participate in process improvements to enable same day appointments and extended hours
- 7Maintain accurate documentation meeting coding guidelines CMS HIPAA standards
- 8Collaborate with Practice Development to achieve PCMH status and NCQA certification
- 9Coordinate transitions of care to specialists and inpatient services
- 10Ensure timely follow up post inpatient discharge
- 11Act responsibly to deliver value through appropriate care at the right time
Requirements11
- 1MD or DO with current Arizona licensure
- 25+ years experience in orthopedic practice
- 3Board Certification in Orthopedics or related field
- 4Fingerprint clearance card required for pediatric behavioral health sites
- 5Bilingual Spanish proficiency preferred
- 6Current CPR and BLS certification
- 7Ability to work flexible day shifts
