Oncology Young Adult Social Worker, LMSW
Overview
You will own psychosocial care for young adult oncology patients at UT Southwestern's Simmons Comprehensive Cancer Center, a NCI-designated facility ranked #1 in Dallas-Fort Worth. You will collaborate with a multidisciplinary team across outpatient and inpatient settings, managing complex care transitions and crisis interventions. This role stands out for its focus on young adults, with opportunities to lead patient programs and shape support services. You will work primarily business hours with some planned evenings and weekends.
What You'll Do9
- 1Participate in multidisciplinary rounds to assess discharge needs, including transportation, DME, LTAC, SNF, home health, and hospice.
- 2Apply crisis intervention skills to help patients and families manage acute medical and psychosocial challenges.
- 3Maintain a working knowledge of community resources and connect patients to appropriate agencies for ongoing support.
- 4Document all assessments, referrals, and tracking in compliance with UTSW standards.
- 5Report cases to governmental agencies as required by law and institutional policy.
- 6Communicate effectively with healthcare providers, patients, and families to coordinate care.
- 7Develop and lead educational sessions for staff, patients, and community partners.
- 8Track post-discharge outcomes monthly and use data to improve transition of care.
- 9Identify gaps in services and propose solutions to leadership for young adult oncology patients.
Requirements8
- 1Master's degree in Social Work from a CSWE-accredited program.
- 2Current LMSW license from the Texas State Board of Social Work Examiners (temporary license accepted for first 90 days).
- 32+ years of related social work experience in healthcare or oncology settings.
- 4Experience with crisis intervention and care coordination.
- 5Knowledge of community resources and referrals.
- 6Strong documentation skills with attention to detail.
- 7Ability to work flexible hours, including up to 20 hours per month of evenings and weekends.
- 8Understanding of performance improvement principles and participation in quality initiatives.
