Social Worker LMSW Emergency Room, Houston Methodist
Overview
You will own comprehensive psychosocial assessments and discharge planning for high-acuity and multi-visit patients in the Emergency Room at Houston Methodist Baytown Hospital. You coordinate with physicians, case managers, and the care team to remove barriers to safe, timely transitions. You hold joint accountability for patient throughput and lead service-line psychosocial initiatives. Your LMSW license and mastery of complex community resources drive measurable improvements in patient outcomes and length of stay.
What You'll Do9
- 1Complete psychosocial assessments and diagnose mental health and substance abuse conditions for ER patients, including victims of abuse and neglect.
- 2Lead daily Care Coordination rounds, identifying at-risk patients and resolving barriers to efficient throughput.
- 3Develop and execute complex discharge plans, coordinating post-acute services and community resources for unfunded and high-risk patients.
- 4Provide brief, goal-directed counseling to patients and families to ease transitions and improve coping.
- 5Collaborate with case managers and the interprofessional team to manage cost-effective care and reduce length of stay.
- 6Educate physicians, nurses, and staff on psychosocial impacts, community resources, and discharge planning methods.
- 7Facilitate transfers to appropriate levels of care, leveraging knowledge of payer rules and Medicare/Medicaid eligibility.
- 8Document all assessments, interventions, and outcomes in the EHR to meet compliance and quality standards.
- 9Serve as a preceptor and mentor for new social workers, promoting professional growth and best practices from NASW standards.
Requirements8
- 1Master's degree in social work from an accredited university.
- 23+ years of social services experience in a healthcare setting.
- 3Active LMSW license from the Texas Department of Licensing and Regulation.
- 4Magnet ANCC recognition in Case Management is required.
- 5Demonstrated knowledge of community resources, payer issues, and government programs.
- 6Proven critical thinking, negotiation, and mediation skills in complex clinical situations.
- 7Proficiency in Microsoft Office Suite (Access, Excel, Outlook, PowerPoint, Word).
- 8Ability to work independently and make sound judgments in high-pressure settings.
