What You'll Do9
- 1Build discharge plans and coordinate interdisciplinary care within 72 hours of admission
- 2Develop individualized social services plans integrated with clinical care
- 3Conduct psychosocial assessments and identify barriers to safe discharge
- 4Facilitate family counseling sessions using empathetic communication strategies
- 5Maintain accurate documentation of interventions and compliance requirements
- 6Act as patient advocate during care plan discussions and family conferences
- 7Coordinate community resource referrals and follow-up care coordination
- 8Update interdisciplinary care plans weekly based on team input
- 9Lead efforts to bridge medical and behavioral health services for complex cases
Requirements9
- 15+ years experience in medical social work or case management
- 2Bachelor’s degree in Social Work required
- 3Active Texas Social Work license mandatory
- 42-5 years experience in pediatric or behavioral health settings preferred
- 5Familiarity with EMR systems and documentation standards
- 6Strong knowledge of insurance and community resources
- 7Excellent crisis intervention skills
- 8Ability to thrive in fast-paced hospital environment
- 9Proven leadership in interdisciplinary teams
