Office Coordinator, Home Health Agency Scheduling
Overview
You will own daily administrative operations for a home health agency with 30+ active patients, coordinating aide and nurse schedules while ensuring compliance with home health care rules. You will act as the central hub between case managers, field staff, and family members, managing patient charts, EVV data, and 485 forms. This role directly impacts patient care continuity by keeping schedules accurate and communication seamless. You will also support the aide hiring process and patient intake, making you essential to the team's daily function.
What You'll Do11
- 1Field incoming calls and greet visitors, answering questions from applicants, case managers, and family members with a cooperative attitude.
- 2Organize and maintain patient charts, supporting chart audits to ensure completeness and compliance.
- 3Manage aide call-offs by quickly finding replacements and updating schedules to avoid gaps in care.
- 4Send, track, and file 485 forms executed by the DON and physicians' offices, ensuring timely processing.
- 5Collect, audit, and file timesheets against care plans and service plans for accuracy.
- 6Oversee EVV data entry and validation to meet state and agency requirements.
- 7Support the aide hiring process by coordinating interviews and paperwork.
- 8Maintain the email inbox and respond to inquiries, flagging urgent items for management.
- 9Prepare the upcoming recerts list for weekly staff meetings and notify the DON or field nurses to schedule visits.
- 10Manage the referral portal and referral emails from case managers, tracking incoming patients.
- 11Track patient hospitalizations and communicate status updates to the team.
Requirements8
- 11+ year of administrative coordination experience in a home health or healthcare setting.
- 2Strong organizational skills with the ability to manage multiple priorities and deadlines.
- 3Proficiency with Microsoft Office (Outlook, Excel, Word) and electronic health records (EHR) systems.
- 4Knowledge of home health care regulations and EVV requirements.
- 5Excellent written and verbal communication skills for interacting with case managers, nurses, and families.
- 6High attention to detail for accurate chart maintenance and timesheet auditing.
- 7Ability to work onsite in Columbus, Ohio with a reliable schedule.
- 8High school diploma or equivalent; associate's degree preferred.
