Occupational Therapist OT/COTA - Onsite Phoenix, Part-Time 19 hrs
Overview
Own patient evaluations, therapy plan development, and collaboration with educators to support students with complex needs in a Phoenix school. You will conduct assessments, implement goals, and monitor progress at a school site with a multidisciplinary team. This role emphasizes hands-on therapy, documentation, and staff training to enable student success. The position is onsite in Phoenix, part-time through 12/18/2026.
What You'll Do10
- 1Build and conduct occupational therapy evaluations and assessments
- 2Develop and implement individualized therapy plans and IEP goals
- 3Collaborate with teachers, parents, and multidisciplinary teams
- 4Maintain therapy records and monitor student progress
- 5Train staff on occupational therapy techniques and best practices
- 6Participate in Medicaid in Public Schools MIPS documentation and billing
- 7Serve as a resource for students and families
- 8Support students in achieving educational and developmental goals
- 9Intervene with students with complex needs and or sensory impairments
- 10Ensure compliance with background checks and licensing requirements
Requirements10
- 1Graduation from an accredited occupational therapy program
- 2Active Arizona OT license for OT role
- 3NBCOT certification and applicable Arizona licensure for COTA
- 4Minimum 2 years of occupational therapy experience
- 5Pediatric therapy experience and experience with complex/sensory needs
- 6Ability to pass state and FBI background checks
- 7Arizona Fingerprint Clearance Card
- 8CPR/BLS certification
- 9Arizona OT license
- 10TB skin test or negative result
