Occupational Therapist, Long-Term Care & Rehabilitation
Overview
You will assess and treat patients in a skilled nursing and rehabilitation center, driving recovery from day one. You own the full therapy cycle: evaluation, treatment planning, and discharge. You will work with Occupational Therapy Assistants and collaborate with physicians and nursing staff. This role centers on geriatric care and long-term rehabilitation, where you see patients through their entire journey. You will set achievable goals and document progress in real time, making a direct impact on patient outcomes.
What You'll Do9
- 1Assess referred patients to determine functional capabilities, limitations, and rehabilitation potential within the first week.
- 2Design and implement individual treatment programs with specific goals under physician direction, adjusting as patients progress.
- 3Document patient evaluations, weekly progress notes, re-certifications, and discharge summaries using HIPAA-compliant practices.
- 4Oversee and supervise Occupational Therapy Assistants to ensure consistent, high-quality care delivery.
- 5Educate patients and families on therapy methods and home exercise programs to support recovery.
- 6Collaborate with nursing, physicians, and other therapists to deliver patient-centered care across disciplines.
- 7Maintain compliance with confidentiality, HIPAA, and Federal Residents' Rights in all interactions.
- 8Report to the Director of Rehabilitation and handle assigned duties with professionalism.
- 9Demonstrate clinical excellence in long-term care and skilled nursing settings.
Requirements8
- 1Graduate from an accredited Occupational Therapist college or university.
- 2Hold a valid professional license and registration for the state of Florida.
- 32+ years experience in long-term care or geriatric care preferred.
- 4Proficient in writing evaluations, progress notes, re-certifications, and discharge documentation.
- 5Strong patient service skills and ability to show clinical excellence.
- 6Ability to work onsite in Orlando, Florida with a reliable schedule.
- 7Demonstrate strong leadership and communication skills.
- 8Comply with confidentiality, HIPAA, and Federal Residents' Rights.
