Occupational Therapist Home Health Job in Seattle, WA
Overview
You will own occupational therapy services for home health patients in the Puyallup/Pierce County territory, conducting assessments and creating individualized care plans. You partner with patients, caregivers, and physicians to restore function and independence in the home. Collaborate with a supportive interdisciplinary team backed by Signature Healthcare at Home and the Pennant Group. This role offers the flexibility of part-time/PRN scheduling with a clear focus on patient-centered outcomes.
What You'll Do8
- 1Assess each patient's functional status including muscle function, endurance, visual coordination, and self-care ability within the first 90 days.
- 2Develop individualized treatment plans in partnership with the patient, caregiver, and physician, setting goals for improved function.
- 3Provide patient and caregiver education on adaptive techniques and home safety to minimize hazards and barriers.
- 4Communicate regularly with the physician and other health care practitioners to align the home health plan of care.
- 5Identify equipment needs and train patients on the use of prosthetic and/or orthotic devices as required.
- 6Supervise Certified Occupational Therapy Assistants according to organization policy and state regulations.
- 7Participate in discharge planning to ensure a smooth transition and continuity of care.
- 8Document all assessments, interventions, and progress notes accurately and timely per Medicare and state guidelines.
Requirements8
- 1Graduate from an OT program accredited by ACOTE of the AOTA.
- 2Hold an active Occupational Therapist license in Washington State.
- 3Pass or be eligible for the NBCOT certification examination.
- 42+ years of clinical experience as an Occupational Therapist.
- 5Maintain a current CPR Certification.
- 6Possess a valid driver's license, automobile insurance, and reliable transportation for home visits.
- 7Demonstrate strong written and verbal communication skills.
- 8Ability to work Monday to Friday, 8:00 AM to 5:00 PM, with occasional flexibility.
