Nurse Practitioner Acute Care, Patient Flow & Level of Care Management
Overview
You will own patient flow and level of care management for the Throughput Team at Houston Methodist Hospital, a top-ranked acute-care facility in the Texas Medical Center. Coordinate care for a high-acuity patient population, driving timely discharges and appropriate level transitions across the hospital. Collaborate with physicians, nurses, and case managers in an interprofessional team. The role includes ACLS certification and prescriptive authority, allowing you to manage complex cases autonomously. What You'll Do8
- 1Coordinate timely, appropriate patient care under physician supervision for defined patient populations, focusing on expected outcomes and individualized needs.
- 2Lead interprofessional rounds to align the care team and expedite discharge planning and level of care decisions.
- 3Conduct comprehensive health assessments, including initial, ongoing, and discharge evaluations, to identify normal and abnormal health characteristics.
- 4Develop and implement individualized care plans, initiating interventions and evaluating care outcomes for patients across the throughput continuum.
- 5Perform invasive, diagnostic, or therapeutic procedures within your credentialed privileges to support patient stability and progression.
- 6Drive quality and safety improvements by initiating outcome and safety initiatives, and integrating evidence-based practices into daily workflows.
- 7Mentor nursing staff and serve as a clinical resource for complex patient flow issues, fostering staff development.
- 8Implement department strategies to achieve financial targets through timely documentation, reduced length of stay, and optimized resource utilization.
Requirements10
- 1Master's degree or higher from an accredited school of Nursing and graduation from an accredited Nurse Practitioner program.
- 2Two years of registered nurse, advanced practice, or nurse practitioner clinical experience.
- 3Active RN license in Texas or compact licensure, with permanent Texas license within 60 days if establishing residency.
- 4APRN Acute Care state license and ANCC-recognized acute-care practitioner certification.
- 5BLS and ACLS certification from the American Heart Association.
- 6Prescriptive authority (AP) from the Texas Board of Nursing and DEA license within 6 months.
- 7Demonstrated assessment, clinical, documentation, and analytical skills in high-acuity settings.
- 8Effective communication with patients, families, and the interprofessional team to foster collaboration.
- 9Strong organizational and problem-solving skills to prioritize multiple tasks and role components.
- 10Ability to function independently and exercise judgment in interactions with physicians and care teams.
