Overview

What You'll Do8
- 1Conduct virtual CMRs and TMRs via phone or video, addressing medication questions and adherence barriers.
- 2Recommend medication therapy adjustments to improve outcomes, using clinical guidelines and chronic disease knowledge.
- 3Review prior authorization requests, formulary exceptions, and step-therapy requirements, aligning with clinical guidelines.
- 4Collaborate with prescribers through secure messaging and phone, negotiating medication alternatives when needed.
- 5Perform proactive outreach for gaps in care, focusing on chronic disease management and preventive services.
- 6Support formulary initiatives and assist with clinical rules and medication utilization processes.
- 7Review complex claims data and support PBM-related clinical activities, ensuring accuracy and compliance.
- 8Document every consultation in EHR and PBM systems, meeting HIPAA and state telepharmacy requirements.
Requirements6
- 11+ years of pharmacist experience with a PharmD or BS Pharmacy degree.
- 2Active, unrestricted pharmacist license in at least one of the listed states, multi-state preferred.
- 32+ years of clinical pharmacy experience, with PBM, managed care, MTM, or telehealth experience strongly preferred.
- 4Strong knowledge of chronic disease state management, DUR, and formulary navigation.
- 5Proficiency with telehealth platforms, EHR, and PBM claims systems.
- 6Excellent verbal and written communication skills for patient and provider interactions.
