Clinical Pharmacist, Telephonic Patient Care - Texas
Overview
You will own telephonic outreach to UHC Medicare members, improving medication adherence and drug utilization at scale. You will work with a collaborative pharmacy team focused on clinical and business outcomes. This remote role requires monthly office visits in Houston, TX, and daily presence during the 3-week training period. You will directly impact CAHPS scores and Star Ratings, with medication adherence contributing 14% to the rating.
What You'll Do8
- 1Perform live telephonic outreach to UHC Medicare members to improve health quality outcomes like medication adherence and appropriate drug utilization.
- 2Optimize pharmacy therapy through additional telephonic engagement with providers and pharmacies.
- 3Oversee pharmacy projects from inception to completion, ensuring deliverables meet quality standards.
- 4Provide responses to drug information requests from internal and external stakeholders.
- 5Assess and interpret customer needs and requirements, identifying solutions to non-standard requests and problems.
- 6Coach, provide feedback, and guide team members on patient care best practices.
- 7Improve CAHPS member experience results by targeting detractors and medication adherence barriers through outbound calls.
- 8Document and track member engagement in MS Office tools to monitor outcomes.
Requirements7
- 1Bachelor's degree in Pharmacy or higher.
- 2Current and unrestricted Pharmacist license in Texas.
- 33+ years of experience as a licensed pharmacist.
- 43+ years of patient consultation experience as a licensed pharmacist.
- 5Beginner proficiency in MS Office and Windows-based environment.
- 6Preferred: PharmD degree.
- 7Preferred: Retail or mail order pharmacy experience.
