Medical Assistant, Health Navigator at One Medical
Overview
Health Navigator at One Medical owns patient interactions and supports a primary care team serving a Senior Health population. You will help with vitals, screening, and care coordination for a panel of patients in a modern, integrated clinic. This role emphasizes human centered, empathetic care and removing barriers to access. You will join a collaborative team delivering thoughtful, proactive care at scale and participate in daily practice operations and patient education.
What You'll Do9
- 1Build rooming and vitals collection with age appropriate screenings for patients
- 2Design post-visit care coordination by scheduling specialists and sharing information with care teams
- 3Lead patient education on chronic disease management and motivational interviewing based action plans
- 4Ship preventive care activities and coordinate referrals to ensure members stay up to date
- 5Own daily front desk and clinic operations to maintain a welcoming space
- 6Drive collaboration with providers to monitor panel health and preventive measures
- 7Manage patient communication via phone and email to improve access and experience
- 8Coordinate anticipatory guidance for families with children and seniors
- 9Scale patient support workflows to improve efficiency and outcomes
Requirements7
- 1At least 6 months of experience in a high touch customer service or patient facing role in a healthcare setting
- 2Graduated from an accredited postsecondary program or completed Medical Assistant training from accredited organizations
- 3Medical Assistant certification issued by a certifying organization accredited by NCQA or ANSI
- 4Strong written and verbal communication skills
- 5Experience working on collaborative, diverse multi-disciplinary teams
- 6Proficiency in computer technology including typing and multiple software systems
- 7Able to work full time 40 hrs/week Mon-Fri 8am-5pm in Phoenix, AZ
