LPN Licensed Practical Nurse - Post-Acute Rehab
Overview
You will own medication administration and treatment delivery for a 50+ resident post-acute rehab unit at Bethany Center for Rehabilitation and Healing, a facility known for its post-acute care programs and innovation. You'll work with a dedicated interdisciplinary team of physical, occupational, and speech therapists to drive resident outcomes. This role offers daily pay, paid orientation, and paid training, with shifts available day or night. You'll be a key player in a facility that has set a new standard for LPN care.
What You'll Do9
- 1Administer medications and treatments accurately, following facility protocols and DOH standards.
- 2Coordinate admissions, transfers, and discharges, ensuring seamless transitions for residents.
- 3Document resident care services in the medical record, maintaining accurate charts and department records.
- 4Educate residents and families on treatment plans, rehabilitation progress, and status changes.
- 5Enforce infection control policies and procedures, and uphold all facility safety standards.
- 6Consult with physicians on resident care plans, reporting any changes in condition promptly.
- 7Protect resident confidentiality and privacy in all interactions.
- 8Collaborate with the interdisciplinary team to solve resident problems and improve quality of care.
- 9Deliver post-acute nursing care that aligns with therapy goals and facility standards.
Requirements7
- 1LPN license from an accredited nursing program or equivalent approved program.
- 21+ years of LPN experience in rehabilitation or long-term care settings preferred.
- 3CPR certification and current state license.
- 4Ability to perform physical tasks, including lifting and supporting residents.
- 5Strong clinical judgment and documentation skills.
- 6Familiarity with DOH regulations and infection control practices.
- 7Flexibility to work day or night shifts, full-time or part-time.
