Overview
We seek a dedicated Licensed Practical Nurse to join our post-acute care programs in physical occupational and speech therapies. This role will drive patient outcomes within a historic setting while leveraging advanced clinical expertise.
What You'll Do8
- 1Administer medications and treatments directly
- 2Support admission transfer and discharge processes
- 3Document resident care services through accurate medical records
- 4Educate residents and families about treatment plans and rehabilitation options
- 5Maintain resident confidentiality and adhere to infection control protocols
- 6Collaborate with physicians to develop and update individualized care plans
- 7Solve resident issues by coordinating with interdisciplinary teams
- 8Utilize EHR systems to track patient progress and ensure compliance
Requirements6
- 1Associated nursing degree from an accredited institution
- 2Graduation from an approved LPN program
- 3Minimum 5 years of clinical experience
- 4Proficiency in electronic health record platforms
- 5Strong communication skills for patient and family education
- 6Ability to work independently and as part of a multidisciplinary team
