What You'll Do11
- 1Identify resident care requirements through personal rapport with residents and stakeholders

- 3Create supportive environments providing emotional psychological and spiritual care to residents and families
- 4Establish patient independence by setting care goals and educating residents families on conditions medications and self-care
- 5Measure health outcomes against care goals to recommend necessary adjustments
- 6Solve resident issues using multidisciplinary strategies
- 7Maintain safe workplaces by following procedures and reporting to healthcare support staff
- 8Ensure confidentiality while protecting residents and employees through infection control protocols
- 9Document all care services accurately in resident and department records
- 10Preserve operational integrity by maintaining resident confidence and professional knowledge
- 11Collaborate across health care teams through communication and team problem-solving
Requirements3
- 1Minimum one year experience in long term care settings preferred
- 2Graduate degree from an approved nursing program required
- 3Current state license as a professional LPN must be maintained
