What You'll Do8
- 1Build and perform nursing visits per physician orders and maintain complete medical records
- 2Design and update patient care plans based on assessments and treatment plans
- 3Lead remote and on-site nursing activities to support patient aging in place
- 4Ship accurate documentation, ensure compliance with state licensing and CPR requirements
- 5Own scheduling coordination and travel to multiple locations within service area
- 6Scale patient outcomes through collaborative care with the broader AccentCare team
- 7Diagnose health issues, implement interventions, and monitor patient responses
- 8Drive quality improvement by adhering to care standards and best practices
Requirements5
- 1Graduate of an accredited practical or vocational nursing program
- 2Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
- 3One year of clinical experience and one year of home health experience
- 4Ability to provide nursing visits according to physician orders and maintain medical records
- 5Must possess and maintain valid CPR certification while employed in a clinical role
