What You'll Do8

  • 1Administer medications and monitor health status for residents with high medical needs, following care plans and documenting accurately.

  • 3Collaborate with RN supervisors and Nursing Assistants to coordinate care and adjust approaches based on resident progress.
  • 4Document observations and interventions in real time, ensuring precise records for continuity of care.
  • 5Support behavioral development by modeling positive interactions and reinforcing community living skills.
  • 6Communicate clearly with families, team members, and supervisors, maintaining professional boundaries while building trust.
  • 7Participate in shift handoffs, ensuring seamless transitions for residents and staff.
  • 8Adapt care techniques to each individual's needs, promoting independence and dignity.

Requirements8

  • 1Current LPN license or CNA certification in Minnesota.
  • 21+ years experience with high medical needs and personal cares.
  • 3Prefer 1+ years in a group home setting.
  • 4Compassionate, patient, and respectful with excellent written and verbal communication.
  • 518 years or older with high school diploma or GED.
  • 6Pass a MN DHS Background Study.
  • 7Available to work every other weekend.
  • 8Current driver's license, valid car insurance, and clean driving record.
