AllCare.ai seeks a Korean-speaking Primary Care Nurse Practitioner to deliver on-site care to Assisted Living and RCFE residents. You will own routine and urgent assessments, chronic disease management, and post-discharge follow-ups, with a steady patient volume across partner communities. Work a structured weekday schedule: 3 days on-site with clustered routes, 2 days in-office at our Irvine headquarters. What You'll Do8
- 1Perform routine and urgent assessments for Assisted Living residents, diagnosing and treating acute and chronic conditions.
- 2Manage chronic diseases including hypertension, diabetes, and COPD, adjusting treatment plans as needed.
- 3Conduct post-discharge follow-ups, medication reconciliations, and order labs and diagnostics.
- 4Coordinate care with facility staff, specialists, and families or power of attorney, ensuring seamless transitions.
- 5Document same-day in the EMR to maintain compliance with CMS and regulatory standards.
- 6Communicate fluently in Korean and English with residents, families, and facility staff.
- 7Drive to multiple partner communities per day using clustered routes to minimize travel time.
- 8Spend 2 days per week at the Irvine office for administrative tasks and documentation.
Requirements8
- 1Active CA NP & RN licenses and an active furnishing license.
- 2NPI, DEA, and BLS certifications required.
- 3Geriatrics or facility-based experience preferred.
- 4EMR proficiency required.
- 5Fluent in Korean and English (required).
- 63+ years of primary care experience preferred.
- 7Ability to work onsite in Los Angeles and travel to partner facilities.
- 8Strong clinical judgment and ability to work independently.
