What You'll Do9
- 1Perform comprehensive patient assessments including physical exams medication reconciliation and detailed medical history reviews during home and facility visits
- 2Diagnose acute and chronic conditions and develop individualized treatment plans
- 3Prescribe medications and manage ongoing chronic illnesses
- 4Coordinate referrals and follow-ups with appropriate healthcare providers
- 5Educate patients and families on disease management lifestyle changes medication adherence and preventive care strategies
- 6Maintain accurate and timely documentation within EMR/EHR systems
- 7Conduct follow-up visits to monitor patient progress and adjust care plans as needed
- 8Assess and manage urgent medical concerns during home or facility visits
- 9Participate in community outreach and patient education initiatives
Requirements5
- 1Master’s Degree from an accredited Nurse Practitioner program (Family NP or Adult/Geriatrics preferred)
- 2Active New York State NP license
- 3National Board Certification (AANP ANCC or ONCC)
- 4Bilingual proficiency in Russian or Chinese preferred
- 5Experience with EMR/EHR systems preferred
